APTITUDE-PROXI aims to promote healthy aging among the cross-border rural population of Catalonia and disadvantaged urban areas. The main purpose of the project is to reduce the risk of dependency in this population within the areas of action. To achieve this objective, the following actions will be carried out:

  • Diagnosis of the population's intrinsic capacity: Using digital tools, primary care teams will assess six dimensions of intrinsic capacity (psychological, cognitive, locomotor, vitality, auditory, and visual capacities). Individuals who exhibit alterations in any of these dimensions will undergo a more in-depth evaluation.
  • Community interventions to improve affected dimensions: Multicomponent workshops designed within the ICOPE framework will be developed. These personalized activities will promote healthy eating, physical exercise, social connection, cognitive stimulation, among others.
  • Monitoring the population that participated in the IC diagnosis: This monitoring will allow for evaluating the effectiveness of the multicomponent workshops and adjusting interventions as necessary.
  • Cultural and community activities: Activities will be conducted to involve social services agents, cultural centers, town halls, and associations with community activities. The aim is to support both caregivers and the elderly in rural areas. These activities will help to unify municipalities, improve project dissemination, and foster its continuity.

The project has been co-financed at 65% by the European Regional Development Fund (ERDF) within the framework of the Interreg V-A Spain-France-Andorra Program (POCTEFA 2021-2027). POCTEFA's goal is to strengthen the economic and social integration of the Spain-France-Andorra border area. It focuses on the development of cross-border economic, social, and environmental activities through joint strategies that promote the sustainable development of the territory.
