Request for public information

Fill in the form to request access to the information:

• Write with a word processor or a ball-pen, in capital letters or print letters.
• Give clear contact details: preferably an email address.
• Be sure to indicate which format you want the information to be sent in. This can be PDF, Word or Excel.
• Print the form, sign it and deliver it to the General Registry of Fundació UAB: Edifici Blanc, 1ª planta, Campus de la Universitat Autònoma de Barcelona, 08193 Bellaterra (Cerdanyola del Vallès).
